This form is only for students in the Bachelor of Arts Program.
- If you are enrolled in multiple courses for the semester and want to withdraw from one or more, you should use the Course Withdrawal form.
- If you are looking to withdraw from your courses this semester and not remain enrolled in ANY classes, you must fill out a Leave of Absence form. This form can be found below, labeled "Withdrawal/Leave of Absence Form". Please refer to the instructions below.

- You are required to log in with your Agora Portal credentials to complete this form.
This form may be used to appeal the final grade for a course. Students should review the Woods College's policy on Academic Appeals on the Academic Policies webpage before proceeding with the form. Criteria for grade appeals is detailed within the form.
- If you are looking to pass/fail a course, you can do so on your course registration page. Please refer to this link for Pass/Fail Form Directions on EagleApps.
- You must speak with your Academic Advisor about the implications of passing/failing a course.
This form is for students who were previously withdrawn or otherwise inactive from Woods College and wish to return. You are eligible to request reactivation if your last enrollment was within the past 5 years.
If it has been more than 5 years since you were last enrolled, or if you were academically dismissed, you must reapply for admission.
Woods College Student Reactivation Form
- Use this form to request a Leave of Absence from the entire semester or if you're withdrawing from all courses.
- A Leave of Absence means you are taking a break from attending classes for a semester or more.
- A Withdrawal request means you are requesting to no longer attend classes at Woods College.
- You are required to log in with your Agora Portal credentials to complete this form.

- If you have not actively enrolled in a degree program for a period of five years, you will be considered inactive and will have to reapply to continue in the program.
- If you have been inactive for less than five years, please complete the "Reactivation" form above.
